Basic Coverage
Huntington Hospital provides a basic level of coverage at no cost to you. Basic coverage is effective the same time you become eligible for benefits, provided you are actively at work.

     40% of monthly base pay (maximum-$1,000 per month)

Optional Coverage Levels
    50% of monthly base pay (maximum-$7,500 per month)
     60% of monthly base pay (maximum-$13,000 per month)

You may increase your coverage one level each year during Open Enrollment. If, however, you are not actively working due to illness or injury, any increase in coverage will begin on the date you return to work. Some disabilities may not be covered or may have limited coverage under this plan.
