Employee Eligibility (TOP)
You are eligible to participate in the health, life and disability benefit plans on the first of the month following date of hire or status change, if: |

your position status is full-time or part-time; and
you are scheduled to work at least 40 hours per pay period
|
Dependent Coverage (TOP)
Employees who are eligible for health insurance coverage may also cover eligible dependents. You must enroll yourself and your dependents within the designated period in order for your dependents to be covered. Eligible dependents are: |

Legal spouse
Qualified domestic partner
Unmarried children
Stepchildren
Legally adopted children, or in the process of being legally adopted
Any child for whom you are the legal guardian |
Proof of Relationships (TOP)
You will be required to submit proof of your relationship to dependents
that you elect to cover under any of the health insurance plans.
The following documents are accepted (according to relationship): |

Marriage Certificate
Birth Certificate (must list names of parents)
Legal Adoption Papers (final adoption papers or confirmation of placement for adoption purposes)
Legal Guardianship Papers
Domestic Partner Certification |
Dependent Children (TOP)
Unmarried, dependent children are eligible
for coverage if: |

they live in the United States; and are
less than 19 years old; or
age 19 through
24 and are a full-time student (12 units/semester)

A child over the age of 19 is eligible for coverage if he/she is mentally or physically disabled and incapable of self-support, provided that the disabled child was covered with benefits prior to becoming age 19. |
Domestic Partner (TOP)
Employees may elect to cover their same-sex or opposite-sex domestic partner under the medical, dental, vision and/or dependent life or dependent AD&D plans to which married spouses are entitled, except where prohibited by IRS regulations.
To enroll or add a domestic partner you must complete a Domestic Partner Affidavit form and submit it to the Benefits department. If the partnership subsequently ends, you must notify the Benefits department to terminate coverage within 31 days of the event. |
Qualifying Event (TOP)
The elections that you make during Open Enrollment or at initial benefits eligibility will remain in effect for the entire calendar year. During that time, if your life or family status changes according to the recognized events listed below, you are permitted to revise your benefits coverage to accommodate your new situation.

All changes must be consistent with the qualified life event.
In most cases, you cannot change your benefit plan, but may modify the level of your coverage.
Changes in benefit levels must be completed within 31 days of the event. |
