Employee Coverage Accidental Death and Dismemberment (AD&D) benefits provide a benefit to you or your beneficiary if you die or are dismembered in an accident. AD&D coverage is offered on two levels, basic and optional.
Basic Coverage ($20,000) Huntington Hospital provides a basic level of coverage at no cost to you. Basic coverage is effective the same time you become eligible for benefits.
Optional Coverage (up to a maximum of $500,000) $50,000 1 times your annual base pay 2 times your annual base pay 3 times your annual base pay 4 times your annual base pay
Family Coverage Family coverage is available if you choose one of the optional coverage amounts. The coverage level based on the percentage of your coverage:
Spouse or Domestic Partner Only - 60% of employee’s coverage Spouse and Children - 50% of employee’s coverage for spouse; 10% of employee’s coverage per child Children - 20% of employee’s coverage per child
Your Beneficiary You can designate your beneficiary by completing, signing and submitting a Beneficiary form to the Benefits department. Make sure your beneficiary form is always current.
